HCAN a participant in 100 Million Healthier Lives

HCAN is proud to be a participant in 100 Million Healthier Lives! Through our participation in Communities Joined in Action, HCAN is part of this unprecedented collaboration of change agents who are fundamentally transforming the way we think and act to improve health, wellbeing and equity. The Robert Wood Johnson Foundation recently awarded $6.3 million to the initiative, which will enable 200+ communities and 500+ health care organizations to accelerate their work on population and community health.
