Cincinnati’s most vulnerable residents face significant hurdles to good health outcomes. We developed Health Care Access Now (HCAN) in 2009 to reduce—and eliminate—health disparities.

25% in poverty

Nearly 25% of people in the Cincinnati Metropolitan area live in poverty.

10% living w/disability

More than 10% of our city’s residents live with a disability.

8% without insurance

More than 8% do not have health insurance.

These kinds of conditions can result in increased social risks and needs, which in turn affect a person’s ability to achieve good health outcomes.

HCAN Community-based Care Coordination pairs individual diverse needs with the complicated matrix of community services.

Talbert House offers a broad network of services across Southwest Ohio, with a focus on prevention, assessment, treatment, and reintegration. Since becoming an HCAN affiliate in 2018, this partnership has created meaningful opportunities to strengthen care coordination, intentionally connect services, and improve health outcomes while enabling both organizations to operate more efficiently and maximize resources.
