Community Health Workers (CHWs) coordinate care for program participants from historically disadvantaged communities through:

CHWs build relationships with participants and help them navigate complex systems. This work bridges care gaps, ensuring that program participants receive the best health care possible.
In 2023:
- 34 CHWs provided services through HCAN’s two community-based Care Coordination programs
- 333 pregnant program participants received 1,174 visits
- 333 adults and 133 children received service through the Pregnancy and Family Health Care Coordination program
- 478 adults received 2,169 visits through the Adult Health Care Coordination program
